At Franciscan Children's, our Health Information Management Department, also known as Medical Records, plays a crucial role in maintaining and organizing patient information for both inpatient and outpatient services. We're here to support clinical providers in meeting documentation requirements and fulfilling medical record requests.

Protecting Your Child's Health Information and Privacy

We understand the importance of safeguarding your child's protected Health Information (PHI), which includes details related to their healthcare, identification, treatment, and payment. PHI may be used for treatment, payment, or healthcare operations.

You have the right to request copies of your child's health information or a list of entities that have received their information from us. You can also specify your communication preferences regarding how and where we reach out to you.

Please note that the Health Insurance Portability and Accountability Act (HIPAA) Minimum Necessary Standard guides the use, disclosure, and request of information for payment or healthcare operations. This means we only use or disclose the minimum amount of information necessary for a specific purpose.

Accessing Medical Information via the Patient Portal

Our Patient Portal offers 24/7 access to personal health information and medical records securely from anywhere with an internet connection. Currently, the Patient Portal is accessible to parents and caregivers of children admitted to our Inpatient Rehabilitation and Inpatient Behavioral Health programs. Requesting Copies of Medical Records

The privacy and confidentiality of your child's health information are paramount to us. We are dedicated to upholding the rights of our patients and families.

To obtain a copy of your child's medical record, we require written permission. Please complete, sign, and either mail or fax the Release of Information form, which you can download by clicking here. Alternatively, you can request the form to be sent to you by calling 617-254-3800, extension 1970.

Requesting Medical Records

For Health Care Providers and Continuing Care, Personal Records, Insurers, etc.:

There is no cost for medical record copies requested by healthcare providers or those needed for consultation or ongoing care, as well as records requested for personal records or insurance claims.

For Attorneys:

Copies requested for legal purposes and similar requests will be charged based on the number of pages in the record. You will receive a pre-bill once the page count is determined. Copies will be sent after we receive your payment in the form of a check or money order.

Please note that if payment is not received within forty-five days, the request will be considered canceled, and no information will be copied or sent.
