This aid is accessible to uninsured US residents with income below 400% of Federal Poverty Guidelines. If eligible, you’re billed comparably to insured patients.

Eligibility steps:

1. Apply for relevant government medical assistance programs.

2. Complete, sign, and date our Financial Assistance Application (found below).

3. Provide income verification.

4. Submit your application to the Financial Counseling Department.
